As a convenience to our patients, and to facilitate your arrival at Palm Harbor plastic surgery center, we have made your necessary forms available online. Completing these forms in advance will save you time at your first visit.   If you need assistance with completing these forms, our staff will be available to answer your questions at the time of your visit.

For your initial visit, please print and complete the following forms and bring them to your first appointment: New Patient Forms (4 pages). Please print the Notice of Privacy Practices to review and keep for your personal records.

If you are a new client requesting a Skin Care Consultation, please complete the following forms and bring them to your first appointment: Skin Care Client Information & Financial Policy. Please print the Notice of Privacy Practices to review and keep for your personal records.
