Thank you for choosing our office. As a convenience to our patients, and to facilitate your arrival at East Lake Plastic Surgery Center, we have made several critical forms available online. The following forms can be downloaded to your computer and printed at home. By completing these forms in advance, you can save significant time at your first appointment. If you need assistance in completing these forms, our staff will be available to answer your questions at the time of your visit.

For your initial visit, please print and complete the following forms and bring them to your first appointment: New Patient Forms (4 pages). Please print the Notice of Privacy Practices to review and keep for your personal records.

If you are a new client requesting a Skin Care Consultation, please complete the following forms and bring them to your first appointment: Skin Care Client Information & Financial Policy. Please print the Notice of Privacy Practices to review and keep for your personal records.
